Job summary

Ennoble Care in Philadelphia, PA is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative home-based care to a diverse patient panel in the Philadelphia region. This full-time role involves diagnosing, treating, and coordinating with a physician-led team to ensure high-quality, patient-centered care in home settings.

You will perform exams, develop treatment plans, order diagnostics, and collaborate with families and caregivers.

Qualifications

• Board Certified with a valid NP or PA certification in PA.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.
• Strong clinical skills with the ability to assess, diagnose, and treat independently.

Responsibilities

• Provide high-quality primary and palliative care to homebound patients in the Philadelphia area.
• Perform physical examinations and develop care plans.
• Order diagnostic testing as needed and prescribe medications and interventions.
• Communicate effectively with staff and clinical operations across platforms.
• Collaborate with patients, families, and caregivers during visits.
