Job summary

Ennoble Care is seeking an experienced Nurse Practitioner to provide primary and palliative care for homebound patients in Bucks County, PA, on a full-time basis. You will work under physician supervision and care for a diverse patient population with chronic diseases.

Responsibilities include exams, care planning, ordering tests, and coordinating with the interdisciplinary team to deliver patient-centered home care. PA NP certification and PA state licensure are required.

Qualifications

• Board Certified, with a valid NP certification in Pennsylvania
• Strong clinical skills with the ability to treat, assess, and diagnose
• Positive demeanor and strong people skills
• Ability to problem solve and operate independently
• 1-2 years of primary care experience with geriatric patients

Responsibilities

• Provide high quality primary and palliative care to homebound patients in Bucks County, PA
• Perform physical examinations and develop care plans
• Order diagnostic testing as needed and coordinate with the physician
• Foster trust with patients, families, and caregivers during visits
• Attend quarterly medical staff meetings
• Collaborate with the clinical team across platforms to ensure continuity of care
