Job summary

Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to homebound patients in the Philadelphia, PA area on a full-time or part-time basis. You will work under a physician and care for a diverse caseload with chronic diseases.

You will perform exams, develop treatment plans, coordinate with the clinical team, and support families and caregivers to ensure high-quality, compassionate care.

Qualifications

• Board Certified NP or PA in PA
• 1–2 years of primary care experience with geriatric patients
• Ability to work independently
• Strong clinical assessment and decision-making skills

Responsibilities

• Provide high-quality primary and palliative care to homebound patients in and around Philadelphia, PA
• Perform physical examinations, collect histories, and develop care plans
• Communicate effectively with office staff and clinical operations across platforms
• Deliver patient-centered care to a defined panel of patients
• Develop impressions, diagnoses, and treatment plans
• Foster trust with patients, families, and caregivers during visits
• Order diagnostic testing as needed
• Prescribe medications and medically necessary interventions
