Job summary

Ennoble Care is a mobile primary care, palliative care, and hospice provider delivering care to homebound patients in the Philadelphia region. Clinicians visit patients at home, coordinating with physicians to manage chronic diseases and deliver continuous, high-quality care.

The Advanced Provider role requires NP/PA board certification in PA, 1–2 years of primary care experience with geriatrics, and strong independent diagnostic abilities.

Qualifications

• 1–2 years of primary care experience with geriatric patients.
• Board-certified NP or PA with PA state license.
• Strong clinical assessment and independent problem-solving abilities.

Responsibilities

• Provide high-quality primary and palliative care to homebound patients around Philadelphia, PA.
• Perform physical examinations and create care plans based on histories.
• Communicate effectively with clinical operations and staff across platforms.
• Develop impressions, diagnoses and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic tests when medically necessary and prescribe medications.
