Job summary

Ennoble Care in Delaware County, PA seeks an experienced Advanced Provider (NP or PA) to deliver primary and palliative home-based care full-time. You will work under physician supervision to care for diverse patients with chronic diseases in the community.

Responsibilities include performing exams, updating histories, developing care plans, prescribing medications, ordering tests, and collaborating with the care team to ensure high-quality, patient-centered care with an emphasis on independence

Qualifications

• Board Certified in PA with NP or PA license.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred but not required.
• Strong clinical skills with ability to assess, diagnose, and treat independently.

Responsibilities

• Provide high-quality primary and palliative home-based care to patients in and around Delaware County, PA.
• Perform physical examinations, obtain/update medical histories, and develop care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Deliver patient-centered care to a panel of homebound patients.
• Develop impressions, diagnoses, and treatment plans; foster trust with patients and families.
• Order diagnostic tests as medically necessary; prescribe medications and interventions.
