Qualifications

• Board-certified NP with PA licensure in Pennsylvania.
• 1-2 years of primary care experience with geriatric patients.
• Hospice experience is preferred but not required.

Responsibilities

• Provide primary and palliative care to homebound patients in Bucks County, PA.
• Perform physical examinations and update medical histories to plan care.
• Communicate effectively with office staff and clinical operations teams.
• Develop impressions, diagnoses and treatment plans for patients.
• Order diagnostics and prescribe medications as needed.
• Attend quarterly medical staff meetings and collaborate with the care team.
• Provide patient-centered care and consult with families/caregivers as needed.
