Job summary

Ennoble Care is hiring an Advanced Practice Provider to deliver mobile primary and palliative care in the Baltimore, MD area. You will care for a diverse set of patients with chronic diseases, working under physician supervision in a full-time capacity.

You will perform exams, manage histories, create care plans, and collaborate with our clinical operations across platforms to ensure high-quality patient care and a supportive team environment.

Qualifications

• Board-certified Advanced Practice Provider (NP/PA) in Maryland.
• Strong clinical skills to assess, diagnose and treat.
• 1–2 years of primary care experience with geriatric patients; hospice experience a plus.

Responsibilities

• Provide high quality care to primary and palliative patients in and around Baltimore City.
• Assess health by performing exams, obtaining histories, and planning care.
• Communicate effectively with staff across internal platforms.
• Deliver patient-centered care to a defined panel.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic tests as medically necessary.
• Prescribe medications and necessary interventions.
