Powerback Rehabilitation to You in Chicopee, MA is seeking a Licensed Physical Therapist for mobile outpatient therapy serving older adults in homes and community facilities. You will assess, plan, and implement home-based rehabilitation programs aimed at restoring function and independence.

This role offers flexible, part-time hours (8–14 hours per week), competitive pay, and opportunities for mentorship and CEUs.

Qualifications

• Must have a valid Physical Therapy license in the state of practice.
• Master’s degree in Physical Therapy required, or equivalent.
• Ability to travel to patients’ homes/locations in Massachusetts.

Responsibilities

• Provide outpatient rehabilitation services in patients’ homes or community facilities.
• Develop and implement individualized home-based therapy plans.
• Educate patients and caregivers to promote independent functioning.
• Collaborate with the rehabilitation team to coordinate care.
• Document assessments, plans, and progress accurately.
• Ensure safety and quality of care in a home setting.
