Job summary

Powerback is seeking a Physical Therapist to provide in-home outpatient rehabilitation for older adults across Nashua, NH and surrounding communities. This part-time role (18-29 hours/week) offers flexible scheduling and the opportunity to advance skills through mentorship and CEU programs.

Responsibilities include evaluating patients, developing individualized plans, coordinating with the rehab team, training caregivers, and documenting care while promoting safety and independence in the home

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy; or Master’s in PT related field with Bachelor’s in PT; or Bachelor’s in Physical Therapy plus 5 years of PT experience.
• Must have a valid driver’s license or reliable independent transportation to and from patients’ residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Assess needs, develop and deliver individualized physical therapy programs.
• Coordinate referrals to other disciplines as needed.
• Educate patients and caregivers to promote independence.
• Supervise PT assistants and ensure proper documentation.
• Maintain safe home environments and manage equipment.
