Powerback Rehabilitation To You is seeking a licensed Physical Therapist Assistant to deliver in-home outpatient rehabilitation for older adults in Chicopee, MA. The role offers a full-time schedule with a flexible environment across patient residences, independent living facilities, and home-based care.

You will work under a licensed PT, perform PT treatments, and engage families in the care plan while maintaining documentation and contributing to program growth and patient outcomes.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation in home or community settings to older adults.
• Supervise and assist under the Licensed Physical Therapist.
• Maintain timely documentation and adherence to treatment schedules.
• Instruct patients, families and caregivers in PT techniques and home exercise programs.
• Perform assessments and contribute to individualized treatment plans.
• Ensure equipment is clean, safe and reports needs for repair.
• Deliver compassionate, patient-centered care in diverse settings.
