Job summary
Powerback Rehabilitation To You is seeking a Physical Therapist to provide outpatient rehabilitation services in the patient’s home or in independent living facilities in the Saugus, MA area. The role focuses on developing individualized plans to restore function, educate patients and caregivers, and collaborate with the care team.

Qualifications
- Must have a valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
- Master's degree in Physical Therapy required; alternative: Master's in PT related field plus Bachelor's in PT; or Bachelor's in PT with 5 years of progressive PT experience.
Responsibilities
- Provide outpatient rehabilitation services in home or independent living facility settings.
- Develop and implement individualized physical therapy programs to restore, reinforce and enhance function.
- Supervise or consult with PT assistants and support personnel as per licensure requirements.
- Coordinate with rehabilitation team and refer to other disciplines as needed.
- Educate patients and caregivers to promote independence and safety in daily activities.
