Job summary

Powerback Rehabilitation To You in Saugus, MA is seeking a licensed Physical Therapist to join our in-home outpatient team on a PRN, consistent schedule. You will evaluate older adults, design individualized therapy plans, and guide patients and caregivers toward independence in their daily activities.

You’ll work across home and community settings, supervise assistants as needed, and document care and progress per professional standards.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master's degree in a PT-related field in addition to a Bachelor's degree in Physical Therapy; or Bachelor's degree in Physical Therapy plus 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation in home-based settings for older adults.
• Develop and implement individualized therapy plans to restore, reinforce, and enhance function.
• Supervise Physical Therapy Assistants and students as required.
• Maintain timely clinical and billing documentation; participate in meetings and care conferences.
• Coordinate referrals to other disciplines to address comprehensive patient needs.
