Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations across a defined coverage area, visiting patients in their homes and bridging gaps in care. Visits typically last 45 minutes, include medication and medical history review, a physical exam, and optional diagnostic tests.

You will use an iPad with a clinical workflow and may perform virtual visits as needed.

Qualifikationen

• Active license(s) in coverage area(s)
• Able to obtain and maintain multi-state licenses as required

• Be part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
• Be flexible to travel locally and within licensed states
