Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations and bridge gaps in care for patients in their homes.

The role involves 45-minute visits, medication and medical history reviews, physical evaluations, and, when ordered by the health plan, additional tests. Travel locally and within licensed states is required; virtual visits are available as needed.

Competenze

• Active, unrestricted medical license(s) in coverage area(s).
• Multistate licenses (preferred).

Mansioni

• Conduct in-home health evaluations within a 45-minute visit.
• Engage patients and caregivers to review medication and medical history.
• Perform physical evaluations and discuss findings with the patient.
• Coordinate with health plan for any indicated diagnostic tests.
• Travel locally and within licensed states; conduct virtual visits as needed.
