Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations across a defined coverage area. The role involves travel up to 100% of the time, conducting visits that include medication and medical history review, a physical evaluation, and optional diagnostics as per health plan.

A valid MD/DO license and multi-state eligibility are required. Clinicians will use an iPad and clinical workflow, connect with health plans, and coordinate with care teams.

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area.
