Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations across defined areas. Visits last about 45 minutes and include medication/history review, physical examination, and optional diagnostic tests. You’ll travel up to 100% and use an iPad with a streamlined clinical workflow.

Housing and travel may be required. The role emphasizes building trusted relationships, delivering personalized care, and coordinating with health plans.

Formación

• Active, unrestricted license in the coverage area.
• Ability to obtain and maintain multi-state licenses as required.
• Willingness to travel up to 100% across defined coverage areas.

Responsabilidades

• Travel across defined coverage areas as required.
• Perform in-home health evaluations including medication and medical history review.
• Conduct physical evaluations and, if ordered by the health plan, additional diagnostic tests.
• Use iPad and clinical workflow to document visits.
• Provide Signify Health services including diagnostic and preventive tests.
• Be flexible to travel locally and within licensed states.
