Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to perform in-home health evaluations. Visits occur in patients' homes, with a focus on medication/history review, physical assessment, and coordinating care through health plans.

You’ll use iPad-based workflow and may travel locally to licensed states. The role includes virtual visits when needed and the opportunity to contribute to comprehensive Diagnostic & Preventive services.

• Be flexible to travel locally and within licensed states as needed.

• Provide all Signify Health services including Diagnostic & Preventive Services (spirometry, DEE, bone density screening, etc.).
