Qualifications

Licensed Physical Therapist in Texas.
Graduate of an accredited Physical Therapy program.
Strong clinical reasoning and communication skills.
Ability to travel to patients' homes for evaluations and treatments.

Responsibilities

Travel to patients' homes to provide PT services.
Perform comprehensive home-based PT evaluations.
Develop individualized plans of care and measurable goals.
Provide skilled interventions: therapeutic exercises, gait training, balance training, and fall prevention.
Educate patients and caregivers on exercises, mobility, safety, and independence.
Document evaluations, treatments, and progress accurately and timely.
Coordinate with other healthcare professionals as needed.
Maintain compliance with regulations and company policies.
