Job summary

West Coast Wound & Skin Care is seeking a skilled Physical Therapist to provide in-home physical therapy services in the Tucson region. The PT will travel to patients’ homes to perform comprehensive evaluations, develop individualized plans of care, and deliver interventions to improve mobility, safety, and independence.

The ideal candidate will work independently, possess strong clinical judgment, and communicate effectively with patients and caregivers while maintaining thorough documentation

Qualifications

• Current and valid Arizona Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Strong knowledge of anatomy, physiology, kinesiology, biomechanics, and rehabilitation principles.

Responsibilities

• Travel to patients' homes to provide physical therapy services.
• Perform comprehensive PT evaluations and develop plans of care.
• Provide skilled PT interventions including therapeutic exercises and gait/balance training.
• Educate patients and caregivers on mobility, safety, and independence.
• Maintain documentation and communicate with care team.
