Job summary

SCAN Health Insurance is seeking an Advanced Practice Clinician to deliver and coordinate primary care for SCAN members in a remote/field capacity. You will care for seniors, conduct wellness visits, manage chronic diseases, administer vaccines, and lead care plans with a focus on timely access and hospital avoidance where possible.

The role involves travel up to 75% in the assigned area and collaboration with physicians, care managers, and specialists to ensure quality outcomes.

Qualifications

• Master’s degree in Nurse Practitioner or PA program with Oregon licensure or PA-C credential.
• Experience with complex chronic diseases and care coordination.
• Knowledge of CMS guidelines and Medicare Advantage.

Responsibilities

• Provide care to an assigned panel in members' residences, including exams and triage.
• Develop comprehensive care plans with patients, families, and staff.
• Collaborate with case management to ensure timely services.
• Monitor and address quality gaps and document visits.
• Engage with physicians and other health professionals for care coordination.
• Participate in on-call after hours and weekends.
