Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to deliver and coordinate primary care for elderly members in their residences or senior living communities in the Portland area. The role involves exams, vaccinations, diagnostic testing, goals-of-care conversations, and chronic disease management.

You will collaborate with case management and primary care physicians, support care coordination, and proactively address care gaps while maintaining documentation and adherence to HIPAA.

Qualifications

• Master’s degree in Nurse Practitioner or PA program.
• Oregon NP license or PA-C in good standing.

Responsibilities

• Provide and coordinate primary care to SCAN members in their living environment.
• Perform exams, vaccines, and diagnostic tests; conduct triage and follow-up care.
• Develop and implement care plans with patients, families and staff.
• Coordinate with case management for timely access to services.
• Engage with physicians and specialists to ensure continuity of care.
• Document visits per protocol and participate in quality improvement activities.
