Job summary

SCAN Group in Tucson, AZ is seeking an Advanced Practice Clinician to provide and coordinate primary care for SCAN members in residential settings. The role emphasizes preventive care, chronic disease management, and collaboration with a broad care team to keep seniors healthy and independent.

You will conduct exams, administer vaccines, coordinate with case management, and engage in care planning with patients and staff. Travel will be frequent in the assigned area.

Qualifications

• Master's degree in NP or PA; education in adult/geriatric care preferred.
• Arizona NP license or PA-C in good standing.
• Wound care and palliative care certificates are a plus.
• Knowledge of CMS guidelines and ICD-10 coding.
• Willingness to travel 75% of the time; valid driver’s license required.

Responsibilities

• Provide care to an assigned panel of members in their residence; exams, vaccines, triage, and chronic disease management.
• Develop a comprehensive care plan for patients, families, and living-community staff.
• Collaborate with case management to ensure timely access to services.
• Coordinate with PCPs, specialists, hospitals, and community providers for continuity of care.
• Document visits promptly according to practice protocols.
• Participate in an after-hours on-call program and travel as needed.
