SCAN is seeking an Advanced Practice Clinician to provide and coordinate primary care for seniors in their homes and senior living communities. You will perform exams, vaccines, and tests, manage chronic conditions, and collaborate with care teams to ensure timely access to services.

This patient-facing role emphasizes triage, care planning, and ongoing education for members and families, with remote/field work and high travel requirements.

Qualifikationen

• Master’s degree in Nursing or Physician Assistant program.
• Oregon NP license or PA-C certification in good standing.
• Experience with complex chronic diseases (dementia, diabetes, heart failure, COPD).
• Wound care and palliative care certs are a plus.

Aufgaben

• Provide and coordinate primary care to SCAN members in their residences.
• Conduct exams, vaccines, and diagnostic tests; triage to ensure timely care.
• Develop care plans with patient, family, and staff; coordinate services.
• Collaborate with case management for timely access to services.
• Document visits and maintain EMR records; participate in on-call rotations.
