Job summary

SCAN Health Insurance is seeking an Advanced Practice Clinician to deliver and coordinate primary care for seniors in homes and senior living communities across Maricopa County. The role emphasizes exams, vaccines, chronic disease management, and care coordination with a multidisciplinary team.

You will work with PCPs and specialists to promote prevention, quality care, and patient advocacy, with significant travel and telehealth elements throughout the assigned area.

Qualifications

• Master’s degree and licensure required.
• Arizona NP license or PA-C in good standing.
• Experience with complex chronic diseases (e.g., dementia, diabetes, heart failure, COPD).
• Knowledge of CMS guidelines and Medicare Advantage.
• Wound care and palliative care knowledge a plus.
• Ability to identify health care needs and coordinate resources.

Responsibilities

• Provide and coordinate primary care in members' homes and living facilities.
• Conduct exams, administer vaccines, order tests, and perform triage.
• Develop comprehensive care plans with patients, families, and staff.
• Collaborate with case management to ensure timely services.
• Document visits in EMR per established protocols.
• Engage with physicians and care providers to promote prevention and management.
