Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to deliver and coordinate primary care for members in their homes and living communities. You will conduct exams, vaccines, and tests, and develop comprehensive care plans while coordinating with the care team.

Travel up to 75% within the assigned CA area is required. California NP license or PA-C is needed, with geriatric expertise and strong collaboration skills to improve patient outcomes.

Qualifications

• Master’s degree in Nurse Practitioner or PA with geriatric focus.
• CA NP license or PA-C in good standing.
• Knowledge of CMS guidelines and Medicare Advantage.
• Wound care and palliative care certificates a plus.
• Ability to travel 75% in the assigned area with reliable transport.

Responsibilities

• Provide and coordinate primary care to SCAN members in their residence.
• Perform exams, vaccines, diagnostic tests, and care planning.
• Collaborate with case management to ensure timely services.
• Document visits timely and participate in on-call rotations.
• Engage with physicians, specialists, and care teams to prevent hospitalizations.
