Job summary

SCAN seeks an Advanced Practice Clinician to provide and coordinate primary care for members in their residences and senior living communities. You will perform exams, manage chronic disease, and work with the care team to prevent hospitalizations.

In this field-based role, you will travel 75% across Tucson/Green Valley and collaborate with PCPs, specialists, and case management to deliver high‑quality, patient-centered care for older adults.

Qualifications

• Master’s degree in NP or PA program required.
• Current Arizona NP license or PA-C in good standing.
• Experience or training in adult/geriatric care preferred.
• Knowledge of CMS guidelines and ICD-10 coding is a plus.

Responsibilities

• Provide primary care to SCAN members in residence or community settings.
• Coordinate comprehensive care plans with patients, families and staff.
• Collaborate with case management to ensure timely access to services.
• Document visits promptly following practice protocols.
• Engage with physicians and specialists to support care continuity.
• Participate in on-call program after hours and weekends.
• Promote care gaps closure and improve clinical workflows.
