SCAN Health Plan seeks an Advanced Practice Clinician to provide primary care to members in their residences, primarily in senior living communities and skilled nursing facilities. You will perform exams, vaccinate, order tests, and coordinate with the care team to avoid hospitalizations.

You will lead goals-of-care discussions, manage chronic diseases, and ensure timely access to services while traveling 75%+ of the time within the assigned region. HIPAA and CMS knowledge are essential.

Qualifications

• Master’s degree in Nurse Practitioner Certificate OR Master’s from an accredited PA program.
• Arizona Nurse Practitioner license OR Arizona Physician Assistant Certification (PA-C) in good standing.
• Wound Care Certificate and Palliative Care Certificate a plus.
• Knowledge of CMS guidelines and Medicare Advantage Managed Care.
• Comfortable with initiating goals of care and end of life discussions.
• Knowledge of HIPAA requirements.
• Knowledge of and ability to learn basic technological skills (Word, Excel, PowerPoint, Outlook, EMR).
• Travel 75+% in the assigned area; valid driver’s license and reliable transportation.

Responsabilités

• Provide and coordinate primary care to SCAN members in their place of residence (senior living communities or skilled nursing facilities).
• Conduct physical exams and assessments; administer vaccines; order tests; interpret results.
• Initiate goals of care discussions with patients and families; coordinate with care team to ensure timely services.
• Develop comprehensive care plans; monitor and address clinical quality gaps in care.
• Document visits and interactions; participate in on-call program after hours and weekends.

Formation

Master’s degree in Nurse Practitioner or Master’s from an accredited PA program

Arizona NP license or PA-C certification

Wound Care Certificate

Palliative Care Certificate

Knowledge of CMS guidelines and Medicare Advantage

HIPAA knowledge

Willingness to travel 75%+
