Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to provide and coordinate primary care for SCAN members in their homes, often in senior living communities or SNFs.

The role requires a Nurse Practitioner or PA-C with California licensure, plus strong chronic-disease knowledge, care coordination, and EMR documentation. Travel 75%+, on-call duties, and a focus on quality care are essential.

Qualifications

• Master’s degree in Nurse Practitioner or PA program (Adult/Geriatric preferred).
• California NP license or PA-C in good standing.
• Wound Care and Palliative Care certificates a plus.
• Knowledge of chronic diseases (dementia, diabetes, HF, COPD).
• Knowledge of HCC and ICD-10 coding.
• Ability to coordinate with resources and care programs.
• CMS guidelines and Medicare Advantage knowledge.
• Comfort with goals of care & end-of-life discussions.
• HIPAA compliance and patient privacy is required.
• Adaptable, problem-solving mindset and openness to new tech.

Responsibilities

• Provide care to an assigned member panel in their residence (senior living or SNF).
• Conduct exams, vaccines, telephonic triage, and potential urgent visits.
• Order/interpret tests; document in EMR; develop plan with care team.
• Collaborate with case management for access to services.
• Coordinate with PCPs, specialists, and community partners.
• Participate in virtual visits and after-hours on-call rotations.
• Document encounters and monitor quality gaps; close care gaps.
• Engage in Integrated Care Sales events to educate communities.
