Job summary

Porter-Starke Services is seeking a Mobile Crisis clinician to provide in-person crisis assessment, intervention, and stabilization for children and adults in home and community settings across Porter County. The role includes crisis stabilization, referrals to higher levels of care when needed, and coordination with the Community Support Services to support transitions of care.

The position requires flexibility to work a 24/7 schedule, including evenings, nights, and weekends.

Qualifications

• Must be prepared to provide immediate, short-term and evidence-based practices to screen, assess, stabilize, and make referrals.
• Experience in crisis assessment and stabilization in home/community settings is preferred.
• Trauma-informed, strength-based approach and de-escalation techniques.
• Ability to coordinate follow-up referrals for behavioral health, physical health, and addictions care.

Responsibilities

• Must be adaptable and able to work various shifts including days, evenings, nights, weekends and holidays.
• Complete crisis assessments and screenings (e.g., CSSRS).
• Engage in crisis planning and safety planning with individuals and families.
• Build rapport to support engagement, hope and care coordination.
• Follow up in-person, by phone, or telehealth within two days of initial intervention.
• Provide warm hand-offs to identify resource needs and insurance navigation.
• Refer clients to medication management and Case Management when appropriate.
• Act as advocate for client rights and confidentiality.
• Maintain confidentiality and professional standards; collaborate with treatment teams and community partners.
• Other duties may be assigned.
