Porter-Starke Services Mobile Crisis Team seeks a dedicated Case Manager to provide in-person crisis assessment, intervention, and stabilization for children and adults within home and community settings. The team operates 24/7, including days, evenings, nights, and weekends, offering rapid access to support and referrals across settings.

You will perform screening, assessment, and placement referrals to crisis stabilization units, inpatient care, or detox services, using trauma-informed care

Qualifications

• Provide in-person crisis assessment, intervention and stabilization.
• Deliver services in home and community settings using trauma-informed approaches.
• Completing outreach referrals and coordinating care transitions.

Responsabilités

• Adapt to multiple shift schedules including days, evenings, nights, weekends.
• Perform assessments and CSSRS screenings.
• Engage in crisis planning and safety planning with clients.
• Coordinate warm hand-offs and referrals to services.
• Maintain confidentiality and professional standards.
