Qualifications

• High school diploma or GED required with CHW certification or equivalent training.
• Experience in community health work, outreach, or care coordination preferred.
• Knowledge of Medicaid, long-term care, and social determinants of health is desirable.

Responsibilities

• Engage with members in homes, hospitals, provider offices, and community settings.
• Build supportive relationships and accompany members to medical appointments.
• Coordinate transportation, reminders, and childcare as part of care plans.
• Gather demographics, eligibility, and health status information for assessments.
• Support development and implementation of individualized care plans.
• Document activities accurately within Care Management guidelines.
• Travel regularly for member, provider, and community visits.
