Insight Global is seeking a Community Health Worker to operate in home and community settings across the Dayton area. You will work with an interdisciplinary team to identify barriers, coordinate services, gather assessment data, and support members in achieving health goals through education and outreach.

The role involves travel to members' homes, hospitals, provider offices, and community agencies, with direct support from Case Managers to implement care plans and improve access to healthcare

Formación

• High school diploma or GED required.
• Relevant experience and knowledge in community health work, peer support, outreach, social services, care coordination, or similar roles.
• Knowledge of Medicaid, long-term care, waiver services, behavioral health, and social determinants of health.
• 2+ years of paid or volunteer experience working with at-risk populations in community settings and coordinating services.
• Experience working with high-risk, underserved, or vulnerable populations.

Responsabilidades

• Engage with members in homes, hospitals, provider offices, and community settings.
• Build professional, supportive relationships with members and families.
• Accompany members to medical appointments and social service visits when needed.
• Coordinate logistics to support care plan goals (transportation, reminders, childcare).
• Gather member information to support assessments (demographics, eligibility, health status).
• Assist with development and implementation of individualized care plans under Case Manager direction.
• Identify barriers to care and connect members with community resources.
• Provide health education and wellness materials as directed by Case Manager or Team Lead.
• Maintain accurate documentation within Care Management guidelines.
• Collaborate with Case Managers and the interdisciplinary care team.
• Travel regularly for member, provider, and community-based visits.
