Job summary

Insight Global is seeking a Community Health Worker to assist members in Dayton, navigating care and community services. You will work across homes, clinics, and community sites to understand needs and support plans, traveling frequently to visit members and coordinate with care teams.

You will collect information, educate on health resources, and help reduce barriers to care, all while documenting activities per care management guidelines.

Qualifications

• High school diploma or GED required.
• Experience in community health work or related field.
• Knowledge of Medicaid, long-term care, waiver services, behavioral health, and social determinants of health.
• Certification as Community Health Worker or completion of approved training.

Responsibilities

• Engage with members in a variety of settings, including members' homes, hospitals, provider offices, and community agencies.
• Build professional, supportive relationships with members and families.
• Accompany members to medical appointments and social service visits when needed.
• Coordinate logistics that support care plan goals, including transportation and reminders.
• Gather member information to support assessments (demographics, eligibility, health status).
• Assist with development and implementation of individualized care plans under the Case Manager.
• Identify barriers to care and connect members with community resources.
• Support and empower members to manage their health and wellness.
• Provide health education and wellness materials as directed by the Case Manager.
• Monitor member concerns and document within Care Management guidelines.
• Collaborate with Case Managers and ICT team; identify process improvements.
• Regularly travel for member, provider, and community-based visits.
