Job summary

Insight Global is seeking a Community Health Worker to operate in a mobile, community-based role. You will work with an interdisciplinary care team to connect members to healthcare and community resources, educate on health topics, and support goals within their care plans.

The role involves visiting members in homes and community settings, coordinating services such as transportation and reminders, and gathering information for assessments under the direction of a Case Manager.

Qualifications

• High school diploma or GED required.
• Experience in community health work, peer support, outreach, social services, or care coordination preferred.
• Knowledge of Medicaid, long-term care, waiver services, behavioral health, and social determinants of health.

Responsibilities

• Engage with members in homes, hospitals, provider offices, and community settings.
• Coordinate logistics to support care plan goals (transportation, reminders, childcare).
• Assist with development and implementation of individualized care plans under a Case Manager.
• Build relationships with members, families, providers, and community organizations.
• Travel regularly for member, provider, and community-based visits.
