Job summary

Insight Global is seeking a Community Health Worker to support members in homes, healthcare facilities, and community settings. You will identify barriers to care, coordinate services, provide health education, and assist with care plans under supervision.

The role requires travel to member and provider sites and a commitment to empowering clients to improve health, safety, and independence. A valid driver’s license and relevant training are expected.

Qualifications

• High school diploma or GED required.
• Experience in community health work or related roles.
• Knowledge of Medicaid and social determinants of health.
• Valid driver's license and reliable vehicle.
• Ability and willingness to travel.
• Community Health Worker Certification or equivalent training.
• 2+ years of paid or volunteer experience with at‑risk populations.

Responsibilities

• Engage with members in homes, hospitals, provider offices, and community settings.
• Build professional, supportive relationships with members and families.
• Accompany members to medical appointments and social service visits as needed.
• Coordinate logistics to support care plan goals (transportation, reminders, etc.).
• Gather demographic, enrollment, and health information to support assessments.
• Provide health education and support care management.
• Maintain accurate documentation per Care Management guidelines.
• Collaborate with Case Managers and interdisciplinary care team.
• Regularly travel for member, provider, and community visits.
