Job summary

Insight Global is seeking a Community Health Worker to support members in homes, healthcare facilities, and community agencies. The CHW helps overcome barriers, coordinates services, and educates on wellness while documenting activities in Care Management systems.

This mobile role requires reliable travel, strong communication, and ability to build trusted relationships with members and families to advance health goals and connect to resources in Dayton, OH.

Qualifications

• High school diploma or GED required.
• Experience in community health work or care coordination is preferred.
• Knowledge of Medicaid, social determinants of health, and community resources.

Responsibilities

• Engage with members in homes, hospitals, provider offices, and community settings.
• Coordinate services and logistics to support care plan goals.
• Gather information for assessments and care plans and document in Care Management guidelines.
• Provide health education and build supportive relationships with members and families.
• Travel regularly for member, provider, and community-based visits.
