Job summary

Insight Global is seeking a Community Health Worker to join a mobile, community-based care team. You will work directly with members in their homes, healthcare facilities, and through various channels to connect them with resources and support.

The role focuses on addressing barriers to care, coordinating services, and helping members achieve health goals. You will accompany members to appointments, gather information for care plans, and provide health education while maintaining timely

Qualifications

High school diploma or GED required.
Knowledge of Medicaid, long-term care, waiver services, behavioral health, and social determinants of health.
2+ years of paid or volunteer experience in community settings.
Community Health Worker Certification or equivalent training is required.
Ability and willingness to travel regularly.

Responsibilities

Engage with members in homes, hospitals, provider offices, and community settings.
Identify barriers to care; coordinate services and logistics.
Accompany members to medical appointments and social service visits.
Gather information to support assessments and care plans.
Provide health education and support member wellness goals.
Maintain documentation within Care Management guidelines.
Travel regularly for member and community visits.
