Job summary

ACCESS in Dearborn, MI is seeking a Community Health Worker I & II to conduct wellness consultations and SDOH screenings. You will promote preventive care, provide health education, and refer residents to social services in local communities.

Under supervision, you will work with health care providers to improve outreach, navigate services, and support clients in obtaining Medicaid, insurance, and other programs. Travel up to 90% and mobile unit environment.

Qualifications

• High school diploma or GED is required.
• Level I requires 6 months to 2 years CHW experience; Level II requires 2+ years.
• Current CHW certification preferred or willing to complete during onboarding.

Responsibilities

• Conduct SDOH screenings to identify needs.
• Refer clients to community resources and services.
• Conduct outreach to promote health in the community.
• Assist clients in navigating and accessing services.
• Provide health education for chronic disease prevention.
• Perform health screenings (vital signs, HIV, Lead, PoC).
• Document client engagement details.
• Follow up to ensure needs are met.
