You will work in members' homes, facilities, and offices, coordinating services, providing health education, and supporting health goals.

You will identify barriers, gather information for plans, and assist with individualized care plans under supervision while traveling to member sites and staying compliant with Care

Qualifikationen

• Comfortable using Microsoft Office, Microsoft Teams, VPNs, mobile hotspots, electronic medical records, and other digital tools.
• Valid driver's license, reliable vehicle, verifiable auto insurance, and acceptable driving record.

• Engage with members in homes, hospitals, provider offices, or community settings.
• Build professional relationships with members and families.
• Accompany members to appointments and social service visits as needed.
• Coordinate logistics supporting care plan goals (transportation, reminders, childcare).
• Gather member information for assessments and care plans (demographics, eligibility, health status).
• Assist with development and implementation of individualized care plans under the Case Manager.
• Identify barriers to care and help address them.
• Connect members with community and social resources.
• Support members to improve health, wellness, safety, and self-management.
