Job summary

OhioHealth in Ohio is seeking a Community Health Worker to support patients facing non-clinical barriers to engagement in treatment plans and recommendations. You will partner with a multidisciplinary team to identify barriers and connect patients with social services and community resources to address those barriers, with individual consultation and follow-up based on Social Determinants of Health.

Qualifications

High school diploma or GED required.
Community Health Worker Certification by an approved training program or started within 1 year of hire.
Experience working with under privileged populations in the community.
1-3 years of work experience.
Work Shift: Day
Day Scheduled Weekly Hours : 40

Responsibilities

Direct patient support (60%): Meet individually with patients to conduct intake interviews and identify non-clinical barriers; coordinate services and provide basic health promotion education; promote treatment adherence.
Communication with health care team (20%): Document interactions with patients; maintain relationships with community resources; report signs and factors affecting care.
Administrative tasks (20%): Maintain patient records; serve as community liaison; triage calls and manage referrals; support safety and efficiency in duties.
