Job summary

ADP, Inc. in Raleigh, NC is seeking a Case Manager to assess participant needs, coordinate individualized service plans, and advocate for clients within behavioral health, addiction recovery, or IDD settings.

You will collaborate with a multidisciplinary team to develop recovery plans, deliver mobile, community-based support, and connect individuals with essential services while ensuring proper documentation and compliance.

Qualifications

• Bachelor’s degree in Human Services or related field required.
• Minimum of 2 years providing direct support in Behavioral Health, Addiction Recovery, or IDD services; 1 year of Case Management preferred.
• Valid driver’s license and intermediate computer skills required.

Responsibilities

• Team-Based Service Planning: Collaborate with a multidisciplinary team to develop and execute individualized recovery plans.
• Direct Client Care & Community Outreach: Deliver mobile, community-based support to help participants access resources and achieve recovery goals.
• Resource Development & Advocacy: Build relationships with community agencies and advocate for participants.
• Documentation & Compliance: Complete assessments, progress notes, and records in line with standards.
• Coverage: Provide coverage to maintain 24-hour access to Intensive Case Management services.
