  • Valid driver’s license and intermediate computer skills.

Responsibilities

  • Team-Based Service Planning: Collaborate with a multidisciplinary team to develop, review, and execute individualized recovery plans.
  • Direct Client Care & Community Outreach: Deliver mobile, community-based support to help participants access resources and achieve recovery goals.
  • Resource Development & Advocacy: Establish relationships with community agencies and advocate on behalf of participants.
  • Documentation & Compliance: Complete assessments, progress notes, and records in compliance with standards.
  • Coverage: Provide coverage to maintain 24-hour access to Intensive Case Management services.

Bachelor’s degree in Human Services or related field
