Job summary

ADP, Inc. in Memphis, TN is seeking a dedicated case management professional to provide family and individual services, assess needs, plan and link clients to appropriate programs, monitor progress, and advocate for services.

You will conduct monthly contacts, coordinate transportation, maintain clinical records, attend trainings, and conduct home visits for home-bound clients, with a sign-on bonus up to $2,500.

Qualifications

• B.A./B.S. in Social Work or related Behavioral Science with some experience in the mental health field.
• Experience dealing with chronic psychiatric populations and severely mentally disabled.
• Must possess a valid driver’s license and proof of insurance.
• Exposure to inpatient psychiatric care (work or training related).

Responsibilities

• Provide family and individual case management services including assessment, planning, linking, monitoring, advocacy, and referral.
• Prevent unnecessary hospitalizations by routing to suitable alternatives and coordinating services.
• Maintain monthly client contacts and higher frequency for high-risk cases.
• Attend meetings and staffing of cases and ensure timely documentation.
• Schedule transportation for groups and medical visits to link clients with other services.
• Report unusual incidents and provide home visits to monitor progress.
• Provide services to home-bound clients and perform other duties as assigned.
