Job summary

Catholic Charities of the Archdiocese of Washington is seeking a bachelor’s-level Community Support Specialist 2 to work with adults with mental illness. You will collaborate with an interdisciplinary team and provide case management, documentation, and support designed to promote recovery.

Responsibilities include crisis intervention, IRP development, and linking clients to community resources, with in-person or telehealth delivery and regular supervision.

Qualifications

• Bachelor’s degree in social work, counseling, or related discipline.
• If a Social Work degree, Social Work licensure in the appropriate jurisdiction is required.
• If a Professional Counseling degree, Professional Counseling licensure in the appropriate jurisdiction is required.

Responsibilities

• Deliver community support services to identified adults with mental illness.
• Establish productive relationships to promote IRP goals and update IRP.
• Provide interventions in the IRP to each assigned consumer.
• Support consumers during mental health crises as needed.
• Care Point contact with discharged consumers to avoid crisis.
• Develop crisis plan strategies to avoid hospitalizations.
• Assist with substance use disorder challenges and relapse prevention.
• Help consumers develop interpersonal and coping skills for home, school, work.
• Educate and assist with health benefits and community resources.
• Maintain accurate documentation of services in Credible by next business day.
• Attend supervision, trainings, and team meetings.
• Coordinate services with the interdisciplinary treatment team.
• Conduct in-person or telehealth services.
