Catholic Charities of the Archdiocese of Washington seeks a Community Support Specialist I to collaborate with an interdisciplinary treatment team and provide case management for adults with mental illness under DC Mental Health Rehabilitation Services guidelines using a Strengths Based Recovery Model.

You will document all services in Credible by the next business day, participate in weekly supervision, conduct home visits, and help consumers develop and update their Individual Recovery Plans

Qualifications

• GED or HS Diploma.
• 2 years of direct service experience in a social service setting with adults who have mental illness.

Responsabilités

• Deliver community support services to identified adults with mental illness.
• Develop and update Individual Recovery Plans (IRPs) with consumers and collaborate with the interdisciplinary treatment team.
• Provide interventions specified in the IRP and support consumers during mental health crises as needed.
• Maintain accurate documentation of all services in Credible by the next business day.
• Attend supervision, participate in daily rounds, and complete required trainings and meetings.
