Catholic Charities of the Archdiocese of Washington is seeking a Community Support Specialist 1 to work within an interdisciplinary team providing case management and community-based services to adults with mental illness. The role emphasizes a strengths-based recovery model and thorough documentation in Credible EHR.

Responsibilities include IRP development, crisis support, home visits, and collaboration with clinical staff to ensure client-centered care.

Qualifications

• GED or HS Diploma required.
• 2 years of direct service experience with adults who have mental illness.
• Must have valid local driver’s license and clean driving record.
• Ability to learn and use Credible EHR and MS Office.

Responsabilités

• Deliver community support services to identified adults with mental illness.
• Develop and update Individual Recovery Plans (IRP) with consumers.
• Provide interventions per IRP to each consumer.
• Support consumers during mental health crises as needed.
• Conduct regular home visits to assess strengths and needs.
• Document all services in Credible by the next business day.
