Catholic Charities of the Archdiocese of Washington seeks a Community Support Specialist 1 to work with adults with mental illness within an interdisciplinary treatment team. The role emphasizes case management, documentation, and delivering community-based supports in line with Recovery Model principles.

Responsibilities include IRP development, crisis support, connection to community resources, and collaborative care planning with clinicians.

Qualifications

• GED or HS Diploma is required.
• 2 years of direct service experience in a social service setting with adults who have mental illness.
• Must have a valid local driver\'s license and clean driving record.

Responsibilities

• Deliver community support services to identified adults with mental illness.
• Establish productive relationships with consumers to promote achievement of IRP goals and regularly participate in IRP development and updates.
• Provide interventions specified in the IRP to each assigned consumer.
• Support consumers during mental health crises as needed and/or directed.
• Linking consumers to resources in the community.
• Coordinate delivery of services with interdisciplinary treatment team.
