Job summary

Open Sky Community Services in Massachusetts is seeking a Navigation Respite Navigator to provide case management support to adults with complex mental health needs in the community. You will conduct outreach, assessments, and person-centered treatment planning for individuals enrolled in the Mobile Respite program.

Collaborate with DMH funders, build relationships with hospitals, crisis support programs, law enforcement, and providers, review documentation, and maintain required certifications;

Qualifications

• Experience in community-based or inpatient behavioral health is required.
• Experience with adults diagnosed with mental illness/substance use disorders.
• Experience coordinating with community partners and resources.

Responsibilities

• Provide case management support to adults in the Mobile Respite program.
• Engage, assess, and plan treatment with individuals in the community.
• Collaborate with DMH funders and community agencies.
• Review documentation and maintain required certifications.
