Open Sky Community Services is hiring a Respite Navigator to support adults in the community with complex mental health needs. You will provide case management, outreach, and person-centered treatment planning, coordinating with local providers and DMH funders.

Competenze

• High School Diploma or GED required.
• Experience in community-based or inpatient behavioral health required.
• Experience navigating health and substance abuse treatment systems.
• Experience with hospitals, crisis programs, law enforcement, or DMH required.
• Valid driver’s license with reliable transportation required.
• Ability to work independently required.

Mansioni

• Provide case management support to adults in the community.
• Serve as the primary contact for community providers and will conduct outreach, engagement, assessment, and person-centered treatment planning.
• Collaborate with DMH funders regarding referrals.
• Establish and maintain positive professional relationships with funding, referral, regulatory and other community agencies.
• Review and complete all required documentation and acquire/maintain required certifications.
