Job summary

Open Sky Community Services Inc. seeks a Respite Navigator to provide case management for adults with complex mental health needs in the community.

You will connect individuals to housing, finance, employment, transportation, and other resources, acting as the main contact for providers. The role requires strong independent work skills, a valid driver’s license, and experience with mental health/substance use services.

Qualifications

• High School Diploma/GED required.
• Experience in community-based or inpatient behavioral health required.
• Experience with adults with mental illness/substance use disorders and accessing local resources preferred.
• Experience working with community partners (Hospitals, DMH, etc.) required.
• Valid driver’s license and reliable transportation required.
• Ability to work independently and manage caseload.

Responsibilities

• Provide primary point of contact with community providers.
• Conduct outreach, engagement, assessment, and person-centered treatment planning.
• Coordinate referrals and liaise with DMH funders as needed.
• Complete required documentation and maintain certifications.
